Medicham is an incredibly powerful sweeper, having its attack boosted to a maximum of 720 with a Choice Band. However, DP has given us the Choice Scarf, giving Medicham a speed boost that it wanted desperately, allowing it to outspeed many key threats.
Your main attack would have to be the fighting attack of your choice. Hi Jump Kick had an increased power in DP, making it able to take out quite a bit more, but the accuracy is still risky, and with Protect running rampant it's riskier still to use it. Brick Break suffers from a drop in power, but is 100% accurate and doesn't cause you to kill yourself if you hit a ghost switch-in. Focus Punch is for the bold and is just as risky as Hi Jump Kick, but has major payoff if you predict correctly.
If you want, you can run Jolly on the Choice Scarf set as even though you suffer from an attack drop, you hit 426 Speed, allowing you to outspeed even more.

Baton Passing

Tochu said I should include this. This set might be able to surprise anyone who thinks that they can stop Medicham by switching in their Cresselia. The premise is simple: Come in on a Pokémon that is bound to run away, Substitute (or Bulk Up) on the switch and carry on from there. A STABbed Focus Punch coming off 450 Attack is bound to hurt a lot, while passing a Substitute / Bulk Up to something that can beat whatever came out to counter you is going to cause some trouble for the opponent.
The EV spread insures it ties with max speed Adamant Tyranitar if you ever find yourself in the situation that it doesn't or can't run away from you. 156 HP and 58 Defense hit 300 and 200 respectively, while as mentioned before 196 Attack hits 450.

You can use either Leftovers or Life Orb, depending on whether you want more survivability or feel that you're not going to hit the opponent hard enough.

Other Options

Fire Punch can go over one of the elemental punches if you want to hit Forretress / Scizor harder, but I only recommend it if you're using a Choice Scarf as Choice Banded Hi Jump Kick 2HKO's both of them. Calm Mind is an option over Bulk Up if you want but it won't work well as Celebi passes them a lot better. Bullet Punch may be 40 Base Power, but it's still going to hurt coming off 720 Attack, but it's hard to work with unless you use it when your opponent doesn't have Cresselia.

Opinion

Definition of glass cannon. It's exceedingly powerful, but sadly DP has gained quite a few counters for Medicham, so it can be hard to play. It can still prove quite a beast with the correct setup, but it's certainly no Garchomp.

Counters

Cresselia works nicely to counter the Choice Bander. Medicham cannot hit it for super-effective damage, doing around 25% maximum, and runs the risk of being Thunder Waved or Cress just putting a Reflect up. Restalk versions can stall it with Ice Beam/Psychic doing a respectable amount, but an unlucky crit can take off half its health if you don't kill it too quickly.

Spiritomb also works well. It resists Medicham's two STAB attacks, and if Medicham attacks with Hi Jump Kick when Spiritomb comes in, it's taken off half its own health. Spiritomb can also Will-O-Wisp it, hit it with Shadow Sneak or Shadow Ball, or Pursuit it if it tries to flee. Dusknoir works well too, having practically the same movepool, but it's 2HKOd by CB Psycho Cut which is a downside.

If you give Medicham a Scarf, then it becomes quite a bit easier to counter. Forretress can take two Hi Jump Kicks and can threaten paralysis with Zap Cannon or can just Gyro Ball you. This is true for the majority of steels that take neutral damage from Fighting moves. Parasect can take an Ice Punch and Spore you as well. pdsciz pdsciz

Loss of Shadow Ball really hit Medicham hard.

That Baton Passing set is just a little gimmicky for a real mention. Life Orb/Bulk Up Sweeper could do with a mention though.

Didn't read anything, just looked at the sets.
